East Sussex Healthcare NHS Trust is recruiting for two part‑time rehabilitation roles in Eastbourne, Lewes and surrounding areas. The JCR service assesses and treats people in their home to prevent hospital admissions and enable earlier discharges.

You will work in a friendly, integrated team with CPD support and opportunities for professional development. The posts are 30 hours and 26 hours per week, with some weekend work and a pro rata salary.

Qualifications

• GCSE grade 4 or GCE C and above.
• Level 2 functional skills or health care support worker apprenticeship / NVQ level 2 in relevant subject.
• Level 3 apprenticeship for example senior health care support worker or NVQ level 3 in relevant subject or equivalent knowledge and experience.

Responsabilités

• Provide rehabilitation to people in a 7‑day service as part of an integrated team.
• Key work non‑complex clients and respond to urgent referrals.
• Support clients to stay in their own home, promoting independence and quality of life.
• Participate in supervision, training and CPD as part of the team.
