Job summary

HCRG Care Group is seeking a Band 5 Physiotherapist for our Intermediate Care Rehabilitation Service in Luton. You will be part of a multidisciplinary team, assessing and treating patients in their homes and in rehab beds, managing your own caseload and delivering personalised rehabilitation.

You will hold a Diploma or BSc in Physiotherapy with HCPC registration, and participate in ongoing professional development to support improving patient independence and outcomes.

Qualifications

• Diploma or BSc in Physiotherapy.
• Current HCPC registration as a Physiotherapist.
• Evidence of continuing professional development (CPD).
• Knowledge and experience relevant to rehabilitation.

Responsibilities

• Complete patient-centred and holistic physiotherapy assessments.
• Establish meaningful and achievable rehabilitation goals.
• Plan, deliver and evaluate individualised physiotherapy interventions.
• Manage and prioritise your own clinical caseload.
• Work with patients in their homes and in rehabilitation settings.
• Monitor progress and adapt treatment programmes as needed.
• Respond to urgent referrals.
• Provide clear advice to patients, families and carers.
