Job summary

Northumbria Healthcare NHS Foundation Trust in Morpeth is hiring two full-time Support Workers to join the Palliative Care Support Team at Morpeth NHS Centre. You will provide compassionate, end-of-life care in patients' homes, guided by the District Nurse and MDT.

The role requires excellent communication, flexibility to work unsocial hours, and a valid driving licence. Permanent contract, Band 3, salary £25,760–£27,476 pro rata, Agenda for Change.

Qualifications

• Excellent communication skills and ability to liaise with MDT, patients and relatives.
• Flexible, calm and compassionate approach to end-of-life care.
• Willingness to work unsocial hours, weekends and bank holidays.

Responsibilities

• Provide care as planned by the District Nurse for palliative/end-of-life patients.
• Monitor patient condition during visits and report changes to Team Leader or District Nurse.
• Provide emotional, physical and spiritual support to patients and carers.
• Provide nutrition and mouth care as appropriate; support hands-on nursing care.
• Record care in patient records (System One) and maintain high standards of documentation.
