HospiceCare North Northumberland is seeking experienced nursing staff to deliver compassionate end-of-life care within their scope of practice, assessing and planning care, and contributing to MDT reviews.

You will maintain accurate records, support families, manage communications across languages, and work with multi-disciplinary teams. The role emphasises safeguarding, governance, and continuous professional development.

Formación

• Clinical Assessing, planning, implementing and evaluating care provision within your scope of practice
• Monitor and evaluate end of life care plans to measure progress and ensure the effectiveness of any interventions
• Maintaining concise and accurate records of care delivery within care plans and Caring for the Dying Patient documentation
• Maintain a duty of care to patients, carers and colleagues through the exercise of personal and professional accountability
• Assess pain and other symptoms using assessment tools, pain history, appropriate physical examination and relevant investigations
• Provide nursing expertise to patients and contribute to community MDT patient review meetings
