Job summary

New Vision Bradford is seeking an Outreach Recovery Coordinator to deliver high-quality substance misuse treatment directly to service users in their homes or residential settings. You will act as the main care coordinator, conducting screening, asset mapping, assessments, recovery planning and care reviews.

The role requires 2 years’ full-time experience in this field, a driving licence and an enhanced DBS check.

Qualifications

• 2 years’ full-time experience of working with people with alcohol and/or drug problems and/or people with complex and multiple needs within the last 4 years.
• Experience of case management and delivering care coordination in drug and alcohol or related field.

Responsibilities

• Act as the main care coordinator for an assigned group of service users.
• Provide screening, asset mapping, comprehensive assessments, recovery planning and reviews of care.
• Deliver high-quality substance misuse treatment directly to service users in their homes or residential settings.
• Guide service users from entry through to completion of their treatment and recovery journey.
