Job summary

Liquid Personnel is recruiting an Occupational Therapist to conduct home-based assessments, identify functional needs, and support independence within the Advice and Wellbeing Hub pathway. The role involves home visits and some office meetings in Reading, with a short-term locum contract.

Applicants must be HCPC registered and have substantial Adult Social Care experience, with a person-centred approach focused on reducing risk and improving safety.

Qualifications

• Qualified Occupational Therapist with current HCPC registration.
• Significant experience within Adult Social Care.
• Strong skills in home-based assessments and moving and handling.
• Experience assessing for major adaptations and recommending interventions.
• Person-centred approach focused on independence and risk reduction.

Responsibilities

• Conduct home-based assessments to identify functional needs and barriers to independence.
• Undertake moving and handling assessments and recommend interventions.
• Assess for major adaptations and provide recommendations to improve safety and accessibility.
• Identify suitable equipment and support options to maximise independence.
• Deliver Occupational Therapy interventions to reduce risk and delay formal care.
• Work within the Advice and Wellbeing Hub's six-week support pathway.
• Manage assessments and interventions to reduce waiting times.
• Liaise with service users, families and other professionals to achieve positive outcomes.
