Job summary

Active Care Group is seeking a Complex Care Support Worker to provide compassionate, person-centred support that enables clients with complex health needs to live safely and independently at home. You’ll join varied care packages, stepping into both new and existing plans with a focus on reassurance and continuity for families and care teams.

This role offers flexibility with zero-hours contracts initially and shifts including 12-hour day and night options.

• Delivering high-quality personal care and daily living support.

• Supporting existing care teams and modelling best practice.
• Communicating effectively with Care Managers and clinical professionals.
• Assisting with medication, equipment, and care supplies where required.
