Job summary

City and County Healthcare Group Ltd invites experienced nurses to join our Complex Care team, delivering high-quality care to adults in their own homes across Birmingham, Wolverhampton, Walsall and surrounding areas.

You’ll use advanced clinical skills, work Monday–Friday 9am–5pm with paid on-call, and enjoy autonomy, supported by a clinical team, allowing a better work-life balance.

Qualifications

• Registered Adult Nurse (NMC Registered).
• Experience in complex/highly complex care settings.
• Experience with multi-disciplinary teams and training/coaching.

Responsibilities

• Perform comprehensive clinical assessments for clients in home settings.
• Develop and review person-centred care plans.
• Provide clinical supervision and coaching to care teams.
• Visit clients at home to review care delivery and identify improvements.
• Collaborate with families, hospitals, GPs, therapists and Local Authorities.
