Job summary

CCH Group is seeking a passionate Complex Community Nurse to support adults with highly complex healthcare needs in their own homes, across Birmingham and the wider West Midlands. You’ll deliver high-quality, person-centred care while building meaningful relationships with clients and families.

In this field-based role, you’ll enjoy Monday–Friday hours, autonomy to plan your diary, and strong clinical teamwork.

Qualifications

• Registered Adult Nurse with NMC registration.
• Experience in Critical Care, Complex Care, Respiratory, Neurosciences, Spinal Injuries, or Community Nursing.
• Confident managing clients with complex clinical needs.
• Skilled in building relationships with clients, families and multidisciplinary teams.
• Passionate about training and coaching others.
• Experience with Tracheostomy care, Long-term ventilation, Cough Assist, Suction, Enteral feeding.
• A full UK driving licence and access to a vehicle are essential.

Responsibilities

• Conduct comprehensive clinical assessments for new clients transitioning from hospital.
• Develop and update person-centred care plans.
• Provide hands-on training and competency assessments for care staff.
• Offer clinical supervision and coaching to care teams to build confidence.
• Visit clients at home to review care delivery and identify improvement opportunities.
• Collaborate with families, hospitals, GPs, therapists, Local Authorities and ICBs.
• Support governance, safeguarding and quality assurance across caseload.
• Help clients remain safely at home and avoid hospital admissions.

Salary: Up to £43,000 per annum + mileage + paid on-call

Location: Home-based, supporting clients across Birmingham, Wolverhampton, Walsall and the wider West Midlands (linked to our Telford branch)

Hours: Monday to Friday, 9:00am - 5:00pm with participation in a paid on-call rota

Job Description

What you'll be doing

As a Complex Community Nurse, you'll play a vital role in ensuring our clients receive safe, high-quality care while supporting and developing the care teams around them.

A typical week could include:

Completing comprehensive clinical assessments for new clients leaving hospital.

Reviewing and developing person-centred care plans.

Delivering practical, hands-on training and competency assessments for care professionals.

Providing clinical supervision and coaching to build confidence within care teams.

Visiting clients in their own homes to review care delivery and identify opportunities for improvement.

Working closely with families, hospitals, GPs, therapists, Local Authorities and Integrated Care Boards (ICBs).

Supporting clinical governance, safeguarding and quality assurance across your caseload.

Helping people remain safely at home and avoid unnecessary hospital admissions.

This is a field-based role, giving you the flexibility to plan your own diary while working closely with a supportive nursing and operational team.

Qualifications

What we're looking for

We're looking for enthusiastic nurses who are passionate about delivering exceptional community care.

You’ll be:

A Registered Adult Nurse (NMC Registered).

Experienced in Critical Care (ICU/HDU), Complex Care, Respiratory, Neurosciences, Spinal Injuries or Community Nursing.

Confident managing clients with complex clinical needs.

Skilled in building positive relationships with clients, families and multidisciplinary teams.

Passionate about developing others through training and coaching.

Experience with any of the following would be advantageous:

Tracheostomy care

Long-term ventilation

Cough Assist

Suction

Enteral feeding

A full UK driving licence and access to a vehicle are essential.
