Job summary

Blackpool Teaching Hospitals NHS Foundation Trust is seeking a Band 5 Physiotherapist to join our Community Therapy North team in Lancashire. You will deliver person-centred rehabilitation in clients' homes and community settings, developing skills to regain independence and confidence while working within a multidisciplinary team.

You must hold a physiotherapy qualification, be HCPC registered, and have NHS or community rehab experience.

Qualifications

• Essential: Qualification Diploma/degree in Physiotherapy.
• Essential: HCPC registration.
• Experience in NHS/community rehab is desirable.

Responsibilities

• Provide person-centred rehabilitation in clients' homes and community settings.
• Support clients to develop daily living skills, mobility and independence.
• Deliver structured intervention plans and monitor progress.
• Work collaboratively with physiotherapists, OTs, nurses and MDTs.
• Facilitate group-based rehabilitation, wellbeing sessions, or community outreach.
• Promote safety, dignity and empowerment in care.
