Blackpool Teaching Hospitals NHS Foundation Trust is seeking a Band 5 Community Therapy Physiotherapist to deliver autonomous assessment, treatment and discharge planning for clients referred to the service.

The post holder will manage a caseload of patients with complex conditions in the community, provide specialist assessments, prescribe equipment, and work with other health and social care professionals to enable daily living activities. Lone-working in the community is required.

Qualifikationen

• HCPC registration or eligible for HCPC registration.
• Autonomous practitioner with ability to manage a caseload.
• Able to work in the community and lone-work safely.

Aufgaben

• Provide autonomous assessment, treatment and discharge planning for community clients.
• Carry out specialist assessments and treatment, including equipment prescription.
• Coordinate with other health and social care professionals and educate staff and students.
