Job overview

This post offers an exciting opportunity for a Speech and Language Therapist to develop specialist skills in transitional neuro-rehabilitation. The Homerton Transitional Neurological Rehabilitation Unit (HTNRU) is a district level (level 2b), seven bed unit commissioned to rehabilitate people to return to living within the community following neurological injury or exacerbation of long term conditions.

The service is based on a social model of disability and the rehabilitation programme is goal led with a focus on community transition and integration. The unit has close links with the wider Homerton neurological specialist pathway services and is supported by an AHP consultant in Neuro Rehabilitation.

You will have evidence of your commitment to trust values and significant postgraduate experience in rehabilitation, stroke and neurology. You will have excellent organisational and interpersonal skills which will allow you to be an effective team member, demonstrate leadership skills and have a good understanding of trust and NHS wide issues. Main duties of the job

The post holder will be responsible for the SLT clinical management of patients, and also providing clinical leadership and coordination as part of the senior MDT in planning a transitional rehabilitation programme for the patients. There is a strong emphasis on positive risk assessment and management to support service users to return to the community and meaningful activities. The post holder will develop skills and knowledge in working with AAC, complex client management including family support and engagement, cognitive-communication disorders and behaviour management, project working, training and working with community services, supervising junior members of staff including rehabilitation assistants.

Homerton is a centre of excellence for neurological rehabilitation and we actively engage with quality improvement and research across our neurospecialist pathway. HTNRU sits within Homerton’s neurological rehabilitation services including the Graham Stroke Unit, Regional Neurological Rehabilitation Unit (level 1 unit), AHP consultant in Neuro Rehabilitation, Neuro Navigation service, RNRU Outreach, Early Supported Discharge (ESD) for stroke and neurology within the Integrated Independence Team, Adult Community Neurological Rehabilitation Team, and specialist AHP lead for spasticity and neuro Orthotic Clinics.

We have strong rehabilitation services in Hackney, enhanced by close working links between community and acute teams.
