Job summary

University Hospitals Coventry and Warwickshire NHS Trust in Coventry is seeking an experienced Registered Nurse to join the Heart Failure Team. The role spans acute, community and virtual settings to deliver specialist care.

You will provide expert clinical assessment and ongoing management of patients with heart failure, working with the MDT and GPs to ensure safe, effective, and personalised care across multiple locations including UHCW, Hospital of St Cross, and Coventry Community Services.

Qualifications

• Experience with heart failure patients.
• Excellent communication and multidisciplinary working skills.
• A driving licence and access to a car is required for travel between sites.

Responsibilities

• Provide specialist assessment and management of patients with heart failure.
• Support the Heart Failure Nursing Team.
• Develop and implement evidence-based treatment plans.
• Support optimisation of heart failure medication in line with current guidelines.
• Refer patients for cardiac rehabilitation where appropriate.
• Refer patients for cardiac device therapy when indicated.
• Promote patient education, self-management and improved outcomes.
• Liaise with the MDT, GPs, healthcare professionals, patients and carers.
• Promote best practice in heart failure management per guidelines.
