Job summary

University Hospitals Coventry and Warwickshire NHS Trust is seeking an experienced Registered Nurse to join the Coventry and Rugby Heart Failure Team. The role involves specialist assessment and ongoing management of heart failure patients across hospital, community, and home settings.

The successful candidate will work within a multidisciplinary team with GPs and other healthcare professionals to deliver safe, evidence-based care and to promote self-management and improved outcomes for

Qualifications

• Current Registered Nurse with active NMC registration.
• Experience with heart failure patients.
• Good IT skills and ability to use MDT systems.
• Excellent communication and teamwork.
• Driving licence and access to a car for site travel.

Responsibilities

• Provide specialist assessment and management of patients with heart failure.
• Support the Heart Failure Nursing Team.
• Develop and implement evidence-based treatment plans.
• Support optimisation of heart failure medication per current guidelines.
• Refer patients for cardiac rehabilitation where appropriate.
• Refer patients for cardiac device therapy when indicated.
• Promote patient education, self-management and improved outcomes.
• Liaise effectively with the MDT, GPs, healthcare professionals, patients and carers.
• Promote best practice in heart failure management in line with guidelines.
