Job summary

The Methodist Church in Britain is seeking a Specialist Heart Failure Nurse to join the Community Cardiology Team. The role focuses on adults with complex cardiac conditions, primarily in a clinic setting with home visits when clinically required.

A full UK driving licence is essential as travel across the service area is part of the job. Applicants should hold a Level 6 degree or equivalent (or nursing diploma) and be NMC registered.

Qualifications

• Level 6 degree or equivalent, or equivalent experience plus a nursing diploma.
• Heart failure specialist course, or willingness to attend within the first year of post.
• NMC registration is required.

Responsibilities

• Manage a dedicated caseload of patients with heart failure, including assessment, planning, implementation and evaluation of care.
• Identify patient needs, monitor disease progression and intervene promptly.
• Deliver personalised health education and lifestyle advice to patients and carers.
• Help patients recognise symptom changes and self-manage to reduce admissions.
• Collaborate with GPs, consultants and other health professionals to optimise medical management.
• Provide clinic-based assessment and follow-up care; visit patients at home when needed.
• Travel across the service area; contribute to service development; maintain accurate records.
