Job summary

East of England Community Health and Care NHS Trust in North Walsham, Norfolk, invites applications for a substantive Band 7 Heart Failure Nurse Specialist. The post continues a successful pilot aimed at detection, treatment and ongoing care for heart failure patients in the community.

You will work with primary, secondary and tertiary care to deliver high-quality care, with substantial experience in nursing heart failure required.

Qualifications

• Current NMC Registration as 1st Level adult registered nurse.
• Post-registration qualification relating to Cardiovascular Disease.
• Post-registration teaching qualification e.g. ENB 998 or equivalent.
• To have or be working towards first or higher degree.

Responsibilities

• Provide holistic safe and effective care using guideline directed medical therapies.
• Plan and deliver all aspects of patient care in line with NMC Code.
• Lead and support the multidisciplinary team to deliver high-quality care.
