Job summary

Tameside and Glossop Integrated Care NHS Foundation Trust in Ashton-under-Lyne is seeking a Band 6/7 Heart Failure Specialist Nurse to join the Heart Failure Specialist Nurse Service on a 12-month fixed-term/secondment maternity cover. You will provide clinical care, support patients and families, and collaborate across hospital and community settings.

Applicants without specialist heart failure qualifications will be considered for Band 6 with support to gain the necessary expertise; Band 7

Qualifications

Registered nurse with current NMC registration and heart failure qualifications.
Willingness to work across hospital and community settings.
Experience in heart failure service preferred or supported to develop.

Responsibilities

Provide clinical care for a caseload of patients with chronic heart failure.
Collaborate with multi-disciplinary teams in hospital and community settings.
Deliver palliative and end-of-life care where appropriate.
Educate other healthcare professionals and participate in the National Heart Failure Audit.
