• Experience delivering community health programmes.
• Ability to plan, deliver and monitor sessions.
• Awareness of safeguarding policies and DBS where applicable.

Responsibilities

• Set up and manage sustainable Healthy Communities projects that deliver measurable community impact.
• Complete monitoring and evaluation as requested by line manager and compliant with data protection.
• Deliver specific Healthy Community projects (e.g., Exercise Referral, gym-based classes) per agreed parameters.
• Plan, deliver and monitor sessions.
• Demonstrate passion for improving local health.
• Use MS tools.
