Job summary

The Forward Trust is seeking a Health & Wellbeing Practitioner to support adult and young men within the Criminal Justice System, helping them improve wellbeing and reintegrate into their communities in Essex. Qualifications

• Experience within criminal justice, health, wellbeing, substance misuse, mental health, housing or social care is desirable.

Responsibilities

• Managing your own caseload and building trusted relationships with clients.
• Assessing individual needs and developing personalised support plans.
• Delivering engaging one-to-one and group sessions.
• Providing practical advice, guidance and wellbeing interventions.
• Working closely with prison teams, community services and partner agencies to ensure seamless support.
• Recording outcomes and contributing to service performance targets.
• Helping people develop the confidence and skills to lead healthier, more independent lives.
