The Forward Trust in Hertfordshire is seeking a Health & Wellbeing Practitioner to support people in custody with substance misuse and low-level mental health needs. You will tailor interventions using harm reduction and abstinence approaches and manage a caseload with assessments and SMART care plans.

Qualifications

• Significant experience in substance misuse and/or mental health services.
• Ability to build trusting, professional relationships with clients.
• Experience delivering psychosocial interventions in 1:1 and group settings.

Responsibilities

• Provide harm reduction advice, guidance and interventions.
• Complete comprehensive and risk assessments and SMART care plans.
• Deliver structured 1:1 and group sessions on substance misuse and mental health.
• Use psychosocial techniques to support positive behaviour change.
• Support clients to develop recovery goals and life skills.
