Job summary

Wirral Community Health and Care NHS Foundation Trust is seeking a Health Visitor to join the 0-25 Healthy Child service in Knowsley. The role requires strong communication and interpersonal skills, with leadership ability and a flexible, compassionate approach to service development.

You will work within an integrated 0-25 Team to support universal partnership and partnership plus families, delivering the Healthy Child Programme, and engaging in public health activities across communities.

Qualifications

• Experience of managing a Health Visiting caseload
• Evidence of continuing professional development
• Experience of leading the assessment, planning and evaluation of care
• Experience of integrated working
• Ability to use email, internet and word processing software
• Sound knowledge of safeguarding issues

Responsibilities

• To improve health and well-being through identifying health needs, raising awareness of health and well-being, influencing the broader context enabling people to improve their own health.
• To work in partnership with the Local Authority and partner agencies.
• To manage an active caseload. To develop and deliver health care and health promotion using evidence-based practice and professional leadership and management.
• To promote positive health to individuals, families, groups, communities, and relevant agencies.
• To contribute to the development of best practice and facilitate shared learning to develop services and initiatives that meet local needs, in relation to aims and objectives of Wirral Community Health and Care NHS Foundation Trust.
