Job summary

Stockport NHS Foundation Trust is recruiting a Health Visitor to join the Start Well Team in Stockport. You will lead the Healthy Child Programme for children aged 0–5 and manage a mixed caseload across universal, enhanced and specialised pathways, working with GPs, Midwives and Stockport Family partners.

You will operate within an integrated Children’s service, focusing on public health priorities to reduce inequalities, while engaging with family hubs and community networks.

Qualifications

• Registered Health Visitor; HV/SCPHN qualification or nearing completion.
• Evidence of CPD and ongoing professional development.
• Demonstrates ability to translate theory into practice.

Responsibilities

• Lead the Healthy Child Programme for 0–5 years within Start Well.
• Manage a named health visitor caseload across universal, enhanced and specialised families.
• Provide holistic assessment and early help within a Team Around the Early Years model.
• Be accountable for your work and delegate as required within the Start Well Team.
• Contribute to reducing inequalities using community health approaches.
