Job summary

NHS Lanarkshire is seeking a Health Visitor to lead care for children from antenatal to school entry, coordinating with families and partner agencies to develop and monitor a care plan. You will undertake statutory duties and ensure safeguarding concerns are assessed and acted upon in line with professional standards.

Qualifications

• Registered nurse or midwife with a degree-level qualification and active NMC registration.
• Post-registration qualification in Health Visiting/Public Health Nursing and Part 3 of the NMC register.
• Masters level qualification in a relevant nursing area or demonstrated equivalent knowledge via NES modules.
• A driving licence (UK/EU/EEA) is required.

Responsibilities

• Undertake statutory duties for children within your caseload to assess, improve and support wellbeing.
• Develop and monitor a multiagency child’s plan with families and partners to meet needs and outcomes.
• Collaborate with families, agencies and stakeholders to ensure responsibilities are carried out per plan.
• Receive wellbeing concerns from various sources and determine if safeguarding procedures should be initiated.

The Role

The main role for the Health Visitor (HV) is to be the designated named HV for all Children within their caseload from antenatal to school entry age, excluding the unborn child. This requires the HV to commence the child’s planning process. To discharge these responsibilities the postholder will have a responsibility to monitor the child’s wellbeing and assess if a need is identified. They will work in partnership with the child’s family and all involved agencies to produce a child’s plan which will require to be monitored.

In discharging these responsibilities the postholder is responsible and accountable for maintaining clinical, staff and information governance.

In this key role, you will:

• To undertake statutory duties on behalf of NHS Lanarkshire with respect to individual children within your caseload to assess, improve and support the wellbeing of children.
• To work with all professionals involved with the child or young person and their families to assess, analyse, develop and agree a multiagency child’s plan based on the agreed policies and procedures. Implement and monitor the plan to endeavour to ensure the child’s needs are met and desired outcomes achieved.
• To work in partnership with individual children’s families, working with other agencies and key stakeholders to ensure that all parties undertake their responsibilities within the agreed plan.
• The Health Visitor will work from the antenatal period to school entry, but excluding the unborn child, the HV will be responsible for receiving wellbeing concerns from a variety of sources in partner agencies (e.g. police reports) and from any member of the community. The HV will be responsible for ensuring that all information received is analysed and included in the assessment of the child’s wellbeing, needs and potential risks. The HV may through assessment of the information known and from direct contact with the family, decide that the level of concern requires instigation of Child Protection procedures. If the child’s needs are not met the HV will elevate their concerns to their line and or professional manager as determined by professional standards, policies and procedures

What You'll Bring:

• Registered nurse or midwife at degree level or equivalent
• Post-registration qualification in Health Visiting/Public Health Nursing and be registered on Part 3 of the NMC register
• Educated to Masters level in a relevant nursing area, or
• Can demonstrate an equivalent level of knowledge gained through, for example, completion of NES modules on Assessment, Leadership, and Critical Thinking.
• A driving licence is required; this must be a full UK/EU/EEA licence.
