Job description

GLL is seeking a Health Referral Facilitator to operate across the sport and leisure facilities at OneNK Leisure Centre. If you possess the necessary skills and ambition, this is an exciting opportunity to advance your career in the health and leisure industry.

Role Overview

The Health Referral Facilitator will prescribe suitable physical activities and support behaviour change for Medically Referred clients. Responsibilities include setting up and conducting group and individual training sessions, managing the GP Referral programme, processing referrals, maintaining accurate records, and handling data.

Reporting To

The role reports to the Healthwise Coordinator / Health Manager.

Key Responsibilities

1. Ensure the efficient daily operation of the Physical Activity Referral Scheme (PARS), optimizing resource use.
2. Conduct health and lifestyle assessments for referred patients and develop safe, appropriate exercise programs.
3. Serve as the primary contact for patients referred to the scheme.
4. Provide evidence-based, culturally sensitive information on healthy lifestyles.
5. Collaborate effectively with operational teams, wider health teams, and GLL support services.

Candidate Requirements

1. Recognized Level 3 Fitness Instructor or Personal Training qualification.
2. Experience in pre-screening and assessing patients for suitability to exercise programs.
3. Experience in monitoring and progressing individuals through exercise routines.
4. Good understanding of the health and fitness industry.
5. Ability to motivate clients and support behaviour change.
6. Strong time management and punctuality skills.
