About the Role
Our client is a dynamic, multi-award-winning teaching partnership serving patients in the community for over 30 years, in the Woodberry Wetlands region, caring for approximately 12,000 patients.
Main Duties
- Deliver care management within your scope of clinical practice, undertaking tasks such as:
- Phlebotomy, blood pressure monitoring, health checks, weight management, ear syringing, basic wound care, vaccinations, spirometry, long-term condition reviews, health promotion, home visiting, chaperoning, and equipment preparation.
- Accurately document all consultations and treatments in patient records and ensure the clinical computer system is up-to-date.
- Assist with administrative responsibilities, including reporting, recall systems, stock management, and participating in practice meetings.
- Communicate effectively with patients, carers, and team members, respecting communication needs and cultural backgrounds.
- Participate in audits and seasonal projects such as flu campaigns.
- Monitor vaccine storage and maintain cold chain records.
- Support practice policies, standards, and guidelines, and discuss their impact with team members.
Skills and Qualifications
Essential
- At least 1 year's recent experience in Primary Care.
- Good literacy and numeracy skills.
- Willingness to undertake further training and qualifications.
- Clinical skills including phlebotomy, blood pressure measurement, and health checks.
- Empathetic communication skills.
Desirable
- Minimum of 2 years' experience in a primary care environment.
- Care Certificate.
- Evidence of ongoing professional development.
