CLCH seeks a Band 6 Specialist Physiotherapist for the Home First Team, covering Brent and Harrow. You will deliver rapid assessment and short-term rehabilitation, enabling safe hospital discharge and home-based recovery.

You will work in a 7-days-a-week service with flexible early/late shifts, within a multidisciplinary team focused on patient-centered care and timely discharge planning.

Qualifications

  • Diploma/degree in Physiotherapy and HCPC registration are required.
  • Evidence of up-to-date CPD portfolio.
  • Attendance at specialist CPD activities in Rehabilitation, Neurology, Orthopaedics or Older People Rehabilitation.

Responsibilities

  • Provide specialist physiotherapy assessment and treatment plans for complex conditions.
  • Manage varied clinical caseload with advanced reasoning for safe discharge home.
  • Collaborate with multidisciplinary teams to deliver integrated care.
  • Identify clinical risks and ensure timely escalation when needed.
  • Assess and prescribe equipment to support safe mobility and independent living.
  • Triaging referrals and supporting meetings to adapt to service demand.

Are you a passionate Physiotherapist looking for an opportunity to make a real difference to patients as they transition from hospital back home?

An exciting opportunity has arisen for a Band 6 Specialist Physiotherapist to join our Home First Team, supporting patients across Brent and Harrow.

Working within a supportive multidisciplinary team, you will provide rapid assessment and short-term, intensive rehabilitation for patients with complex health and social care needs. You will play an important role in supporting safe and timely hospital discharges, helping patients regain their independence and continue their recovery in the comfort of their own home.

You will manage a varied and complex caseload, working closely with health and social care partners across Brent and Harrow to deliver coordinated, responsive and truly person-centred care.

Our Home First service operates 7 days a week, 8:00am–8:00pm, including bank holidays, with full-time hours worked flexibly across early, late and long-day shifts.

Main duties of the job

You will:

  • Provide specialist physiotherapy assessment and develop evidence-based treatment plans for patients with complex, frailty-related conditions and multiple co-morbidities.
  • Independently manage a varied clinical caseload, using advanced clinical reasoning and person-centred goal setting to maximise mobility, independence and support safe, timely discharge home.
  • Work collaboratively within the multidisciplinary team and with CLCH specialist services, health and social care partners to deliver coordinated, integrated care.
  • Identify and manage clinical risks appropriately, including lone-working considerations, informed consent and timely escalation of clinical concerns.
  • Assess, prescribe and order appropriate specialist equipment to support safe mobility, moving and handling, and independent living at home.
  • Contribute to the day-to-day running of the service, including triaging referrals, participating in/chairing multidisciplinary meetings and responding to changing service demands.
  • Provide clinical leadership, supervision and education to junior colleagues, support workers and students, supporting their ongoing development.

Education/Qualification
  • Diploma/degree in Physiotherapy
  • Current HCPC registration
  • Evidence of up to date CPD portfolio
  • Attendance at specialist short courses or equivalent CPD activities within the specialist areas of Adult Rehabilitation, Neurology, Orthopaedics, Musculoskeletal Outpatients, Older People Rehabilitation
  • Member of Professional Body
  • Member of a Special Interest Group relevant to this post
Experience
  • Experience post registration as a Band 5 physiotherapist
  • Experience working at a junior level in a wide range of specialised clinical areas including outpatients, neurology, rehabilitation, orthopedics and respiratory
  • Experience working with voluntary organisations.
Skills & Knowledge
  • Skills in the assessment and treatment of adults with complex multi-pathologies
  • Knowledge and understanding of complex medical conditions including multipathologies and falls
  • Knowledge of role of Allied Health Professionals and interdisciplinary team working
