St James Practice is a general practice in Beckenham,Bromley providing NHS primary care services to over 7,000 patients. We have awell-established team of 3 GP Partners, 1 salaried GP, 1 GP Retainer, 2Practice Nurses, and 1 HCA.
We are a training practice with medical students and GPregistrars, we are involved in new projects to change the delivery of PrimaryCare. We work in conjunction with our PCN onsite Clinical pharmacists,Dietician, Physiotherapist, Advanced Mental Health Nurse, Nurse Associates, HCAand Health and Wellbeing Coach to expand the care we provide to our patients.We are passionate about staff health and wellbeing and working together as ateam. Purpose ofthe Role
To deliver high-quality, patient-centred nursing serviceswithin the general practice setting. The post holder will promote health,provide clinical care, and support long-term condition management in acourteous and professional manner.
KeyResponsibilities
Clinical Duties
Working within your scope of practice and experience:
Deliver corenursing services including:
Cervicalcytology
Chronicdisease management (e.g. Asthma, COPD, Diabetes)
Wound care
Earirrigation
Conductpatient assessments, develop and evaluate care plans
Home visitswhen required
Promotehealth education and lifestyle advice
Makeappropriate referrals to internal/external health professionals
Support QOFachievement and other clinical targets
Participatein clinical audits and implement improvements
AdministrativeDuties
Maintainaccurate, timely documentation using EMIS Web
Monitor andorder medical supplies; ensure appropriate storage
Supportmaintenance and servicing of clinical equipment
Ensureclinical areas are well-stocked and maintained
Assist withrecall and follow-up systems for patient care
Communication
Buildeffective working relationships across the multidisciplinary team
Attendinternal/external meetings relevant to the role
Communicateclearly and compassionately with patients and carers
Ensureinformed consent is always obtained
Respondappropriately to internal/external communications
Clinical andPN meetings
Quarterlynurse meetings with named partner
