Job summary

The Hertfordshire Partnership University NHS Foundation Trust is seeking a GP Plus Practitioner (Band 6) to work on a bank contract in Stevenage within the Primary Care Mental Health Service.

The role focuses on bridging Primary Care and Mental Health Services, providing assessment support to GPs, and facilitating patient pathways between primary and secondary care. Experience in community mental health and CPD evidence is essential.

Qualifications

• Registered mental health professional appropriate to the job role (nursing, OT, SW).
• Evidence of continuous professional development.

Responsibilities

• Undertake assessments of individuals and their families within primary care including risk assessment for access to secondary mental health services.
• Be a core member of primary care teams within a defined area, using specialist mental health knowledge.
• Develop links and foster a culture of partnership between Primary Care Teams and HPFT Adult Mental Health Services.
• Promote use of treatment protocols and care pathways for greater integration of service users within Primary Care.
• Enable workers in Primary Care to develop skills and confidence in assessment and treatment of mental health problems.
• Facilitate joint work between Primary Care and Secondary Care and voluntary sector to increase services.
