Job summary

Transforming Support Sussex is hiring a Support Worker at Harvest Rise, Littlehampton, to support adults with learning disabilities and complex needs. You will deliver consistent, person-centred support to promote independence, dignity and choice, building trusted relationships and helping with daily living and community activities.

The role is full-time and permanent with a rolling two-week rota; benefits include a discretionary £1,000 annual bonus, 28 days' annual leave and a funded enhanced

Qualifications

• Six months' care or support experience.
• Good verbal and written communication skills.
• Ability to work effectively as part of a team.
• Promote independence, dignity and choice.

Responsibilities

• Provide person-centred support reflecting individual needs, preferences and goals.
• Support daily living and independence.
• Help access local community, appointments and social activities.
• Assist with health needs and medications per procedures.
• Support tenancy, finances and household tasks where required.
• Build positive relationships with the people you support, families and professionals.
• Encourage confidence, independence and positive risk taking.
• Maintain accurate support records and documentation.
• Follow safeguarding, health and safety and risk management procedures.
• Work closely with colleagues to provide consistent support.
