Responsibilities

• Support people in their own homes and in the community, following the care plan.
• Build independence and empower people to manage their lives.
• Assist with personal care (washing, dressing, bathing, toileting) at the person’s pace.
• Prompt, monitor or administer medication and record it properly.
• Prepare meals, assist with shopping and paying bills
• Accompany to outings, hospital and GP appointments
• Keep accurate records and report changes promptly
• Collaborate with unpaid family carers who know the person best
