Job summary

East London NHS Foundation Trust is seeking an experienced Mental Health Practitioner (Nurse) at Band 6 to join a front-door service. You will assess referrals, balance risk, and coordinate care plans to help people access timely support in the least restrictive environment possible.

The role requires autonomy, creative problem-solving, and excellent collaboration with service users, carers, families and partner agencies to promote recovery and independence.

Qualifications

• Registered Nurse Part 3 and 13 with NMC registration or Registered Social Worker with Social Work England.
• Demonstrated evidence of continuing professional development (CPD) and portfolio.
