East London NHS Foundation Trust is seeking a Band 6 Mental Health Practitioner to join a front-door service. You will receive referrals from GPs and acute services, helping people access timely support in the least restrictive environment.

The role involves working directly with service users, carers and partner agencies to assess risk, formulate care plans and mobilise community resources that promote recovery and independence.

Qualifications

• Registered professionals must be NMC registered or Social Work England registered.
• Minimum 18 months post-qualification experience.
• Experience in multi-professional team and inner-city area.

Responsabilités

• Build strong therapeutic relationships quickly and remain calm in complex situations.
• Assess risk, formulate care plans and mobilise community resources.
