Job summary

Provide CIC in Chelmsford is seeking a Band 7 Nurse to join the Frailty Virtual Ward. The post involves non‑medical prescribing, comprehensive geriatric assessments, and consultant collaboration to manage care in patients’ homes and reduce hospital admissions.

The role is full‑time (37.5 hours), permanent, with a 7‑day pattern (8am–4pm or 12pm–8pm) and ample learning opportunities within a multidisciplinary team.

Qualifications

• Current registration with professional body relevant to their profession.
• Qualified Independent Prescriber / willingness to attend training.
• Management education development, including supervision/teaching/team management.
• ACP accredited course.

Responsibilities

• Undertake comprehensive geriatric assessments and formulate personalised management plans with MDT input.
• Attend rota for virtual GP calls and liaise with Frailty Consultant for complex cases.
• Lead on clinical supervision of staff and support junior colleagues for competencies; manage discharges with MDT.
• Drive service improvements and innovative approaches to admission avoidance and patient flow.
