Here at the frailty virtual ward we are looking to recruit a Band 7 Nurse, who has a non-medical prescribing qualification and assessment and consultation course,to come and join our service. The applicant needs to have had at least 3 years of community experience and an extensive understanding of frailty. The post holder will help to drive the team forward, and support by delivering assessments in the patient's normal place of residence. The service works closely with Urgent Community Response Team (UCRT) in helping to prevent unnecessary admissions to hospital and supporting patients to remain at home.
The service is based at Kestrel house in Chelmsford and are co located with UCRT and the district nurses for the area. Multi-disciplinary team meetings are held twice weekly and there are great opportunities for further learning around Frailty, with great support from consultants who work within the team.
