Job summary

South Tees Hospitals NHS Foundation Trust is seeking a Service Lead for Transfer of Care / Frailty at Band 8a. The role drives safe, high-quality, evidence-based clinical services across the Unit and coordinates with Operations Managers, Nursing leaders, and Clinical Directors to deliver patient care within budget constraints.

The post requires leadership of team leaders and senior staff, development of service strategy, and collaboration across the local health system to meet NHS priorities.

Qualifications

• Degree or Diploma AHP / Social Work.
• HCPC / BASW registered.
• Evidence of recent professional development.

Responsibilities

• Lead and manage Services within the Transfer of Care Directorate.
• Contribute to medium/long-term strategy for services.
• Ensure safe staffing levels and monitor expenditure.
• Provide leadership to senior staff and teams across multiple sites.
