Kent Community Health NHS Foundation Trust is seeking a Registered Pharmacy Technician to join the Frailty Urgent Care Services team, delivering hospital-level care at patients’ homes. The role covers medicines reconciliation, review and counselling, delivered remotely and face to face, with multidisciplinary board rounds and liaison during transfers of care.

You will oversee medicines stock management, identify priority patients for pharmacist review, and support patient consultations.

Qualifications

• Post-qualification medicines optimisation experience in a hospital or clinical setting.
• Experience training colleagues and mentoring others.
• Experience undertaking audits and contributing to quality improvement.

Responsabilités

• Perform medicines reconciliation, review and counselling remotely and face to face.
• Attend multidisciplinary board rounds and support transfers of care.
• Assess medication adherence and implement interventions.
