Buckinghamshire Healthcare NHS Trust is seeking an Advanced Care Practitioner (Band 8a) to join the expanding frailty team at Stoke Mandeville Hospital. The role offers 37.5 hours per week, including nights and weekends, with flexible options for full-time, part-time or job-share.

The ACP will provide autonomous clinical support for frailty assessments, rotate across Stoke Mandeville and community sites, and contribute to a 24/7 frailty care model delivering proactive and preventative care for

Formación

• First-level degree (HEI Level 5 or 6) or equivalent in the undergraduate base profession
• NMC or HCPC registration
• Masters in Advanced Practice or equivalent
• Accreditation as an Advanced Practitioner or willingness to work towards (NHSE digital badge)
• Qualified as a non‑medical prescriber
• Evidence of continuing professional development (CPD)
• Post‑graduate management/leadership training
• Qualification in teaching

Responsabilidades

• Provide autonomous clinical support to frailty health assessments and discharge planning
• Rotate between Stoke Mandeville Hospital and community sites (Amersham and Buckingham Hospitals)
• Support care delivery oversight within the community in‑patient ward and undertake independent outpatient clinics
• Work in a 24/7 rotating shift pattern across a developing 24hr frailty model of care
