Wakefield Council is seeking an experienced Social Worker to join our specialist Forensic Social Work Team within a secure mental health inpatient service. You will play a key role in supporting service users, their families and carers throughout their care pathway.

You will collaborate with multidisciplinary clinical teams and partner agencies to deliver high-quality, person-centred interventions that promote recovery, social inclusion and safe community reintegration.

Qualifications

• Experience in forensic mental health settings.
• AMHP qualified or eligible for AMHP status.
• Senior social worker level readiness for complex caseloads.

Responsabilités

• Deliver high-quality forensic social work interventions and psychosocial support.
• Support service users and families throughout the care pathway.
• Coordinate and contribute to discharge planning from admission to community transition.
• Collaborate with health colleagues, local authorities, criminal justice agencies and community services.
• Contribute to multidisciplinary meetings, care planning reviews, risk management and case formulations.
• Promote service user rights, safeguarding, recovery and social inclusion.
• Support statutory social work functions and ensure compliance with policies and legislation.
