Job summary

NHS Greater Glasgow and Clyde is recruiting for a Flow Navigation Centre Registered Nurse (Band 5) at the Queen Elizabeth University Hospital, Glasgow. You will deliver a telephone-based clinical service, triage and refer to appropriate care settings, and work within a MDT to optimise patient flow.

Qualifications

• Current NMC registration as a Registered Nurse.
• Evidence of continued professional development.
• Strong clinical decision-making skills to support safe and effective practice.
• Highly developed communication, listening, and interpersonal skills.
• Ability to work in a fast-paced, digitally enabled environment.
• Excellent team-working and initiative.
• Excellent time management and organisational abilities.
• Working knowledge of basic IT systems.

Responsibilities

• Receive and manage referrals via telephone from GPs and other professionals.
• Triages using clinical pathways to direct patients to appropriate care.
• Follow up Did Not Attend (DNA) cases to ensure continuity of care.
• Collaborate with Emergency Departments and other health and social care teams.
• Assess and triage patients to ensure care is in the right place at the right time.
• Demonstrate effective communication and listening to respond to patient needs.
• Maintain a patient-centered approach with professionalism and compassion.
• Follow relevant SOPs, policies and guidelines.
