Job summary

Qualifications

• Experience in health or social care preferred.
• Ability to work in community settings and clinics.
• Willingness to undertake phlebotomy, ECG and PHM training.

Responsibilities

• Carry out routine physical health observations (BP, pulse, temp, height, weight, BMI).
• Support venepuncture pathways and blood sampling.
• Assist with ECGs and clinical monitoring
• Promote healthy lifestyle interventions and PHM model delivery
• Record results and maintain electronic health records accurately.
