Hampshire and Isle of Wight Healthcare NHS Foundation Trust is seeking an experienced Physical Health Specialist to join the Early Intervention in Psychosis service. You will deliver physical health monitoring, health interventions and tailored wellbeing programmes, working with service users and a multidisciplinary team to reduce health inequalities.

Responsibilities

  • Undertake physical health checks following SHFT and national guidelines.
  • Contribute to data collection and analysis for service evaluation and policy compliance.
  • Facilitate health promotion and education to service users and staff.
  • Support colleagues in completing physical health checks, including outreach to those hard to engage.
  • Ensure physical health monitoring is aligned with best practice and mortality reduction.

Are you an experienced Support Time and Recovery Worker passionate about improving the physical health of people experiencing mental health difficulties?

We are looking for a motivated, enthusiastic individual to join our Early Intervention in Psychosis (EIP) service team as a Physical Health Specialist, a unique development opportunity for those with significant mental health experience and physical health skills, particularly in phlebotomy, health promotion and supporting lifestyle change.

You will work closely with service users and the multidisciplinary team to deliver physical health monitoring, health interventions and personalised wellbeing programmes, helping to reduce health inequalities and improve outcomes for people with severe mental illness., Understand the processes of physical health monitoring in EIP teams.

Responsibilities
  • To implement health coaching and physical activity interventions and lifestyle modification to support service users to make life changes to promote better health outcomes.
  • To undertake physical health checks using processes that follow SHFT and national guidelines.
  • To contribute to the collection and analysis of information relating to physical health monitoring and compliance with policy to support service evaluation and improvement.
  • To facilitate and/or provide health promotion and education to service users and staff.
  • To support colleagues within the EIP team to complete physical health checks, particularly with those service users who are difficult to engage or are in isolated locations with limited public transport.
  • To carry out any duties as specified to ensure physical health is monitored in line with best practice and to reduce the mortality gap.
  • To ensure that physical health monitoring outcomes are communicated with primary care in a timely manner.
  • To use ICE or relevant systems to request testing to ensure timely monitoring and results being communicated effectively.
  • To collaboratively create and deliver personally tailored physical health action plans to service users in the team.
  • Liaise with a wide range of internal and external stakeholder to develop an understanding of available physical health interventions.
Qualifications
  • Have substantial experience working in mental health services.
  • Are confident engaging with service users with complex needs.
  • Have phlebotomy skills or are willing to undertake training.
  • Have an interest in physical health, fitness, wellbeing, and health promotion.
  • Are committed to professional development.
