Job summary

Hafod Housing Association Limited in Wales seeks a Support Worker to assist older adults, including those with mental health needs or physical disabilities, to live independently and maintain tenancy.

This 18 hours per week permanent post requires a caring, person-centred approach and a full driving licence. You will work across Golau Caredig and clients in their homes, developing personalised plans and coordinating with partners to achieve positive outcomes.

Qualifications

• Caring, compassionate and person-centred approach.
• Experience with people who have complex needs, including mental health or disabilities.
• Good communication, interpersonal and organisational skills.
• Ability to work independently, manage workload and maintain records.
• Understanding of housing, welfare benefits or community support services is advantageous.
• Full driving licence is essential for the role.

Responsibilities

• Provide flexible, targeted housing-related support to residents at Golau Caredig and in the wider community.
• Support tenancy sustainment, budgeting, accessing services, education and employment opportunities.
• Develop and review person-centred support plans and risk assessments.
• Collaborate with external agencies and partner organisations for coordinated support.
• Maintain accurate records, reports and documentation in line with policies.
