Job summary

Mid Cheshire Hospitals NHS Foundation Trust is recruiting a Band 5 registered nurse to join the Community Care team. You will assess nursing needs, implement and evaluate care plans, and deliver high quality holistic care in the community and wound care clinics.

Travel across the CCICP footprint is required. The role requires strong communication, adaptability and time management, with a structured induction and mentorship during the probation period.

Qualifications

• Registered nurse with current NMC registration.
• Experience in community care or district nursing preferred.
• Ability to work across CCICP footprint and flexible to cover multiple sites.

Responsibilities

• Assess health needs and develop care plans for identified populations.
• Deliver holistic care in community settings and wound care clinics.
• Maintain accurate records on Emis Web.
• Collaborate with other healthcare professionals to ensure delivery of services.
• Provide education and support to patients and carers.
