Job summary

Mid Cheshire Hospitals NHS Foundation Trust in Alsager is seeking a Band 5 registered nurse to join our community care team on a part-time basis (30 hours/week, including weekends). The role involves wound care clinics, home visits, and care planning to support local patients across the footprint.

You will work with a mentor and receive a structured induction. We offer flexible working, a robust induction, and opportunities for local development within CCICP.

Qualifications

• Registered nurse with current NMC registration and community experience.
• Ability to assess, plan, implement and evaluate nursing care in the home.
• Strong communication and collaboration with multi-disciplinary teams.

Responsibilities

• Assess, plan and evaluate holistic nursing care for identified patients in the community.
• Hold a designated caseload and deputise in the absence of the District Nurse.
• Liaise with healthcare professionals to ensure effective service delivery.
• Document patient contacts on EMIS Web and maintain confidentiality.
• Provide wound care, leg ulcer management, IV medications and catheter care.
