Hull City Council is seeking a part-time, 20-hour-per-week role in the Adult Social Care team. The post is permanent, Grade 6 – Pro Rata, with a salary of £16,494 to £17,902. The successful candidate will work as a first point of contact within a multi-disciplinary team to support vulnerable adults and promote independence.

Applicants must have the right to work in the UK. The role requires flexible, occasional weekend work and involvement in the Integrated discharge and Discharge to Assess

Qualifications

• Ability to engage in solution focused conversations with individuals and families.
• Provide information and advice in line with statutory obligations and primary legislation.
• Develop proportionate support plans and refer to appropriate services.

Responsabilités
