Job summary

The Street Lane Practice is seeking a First Contact Practitioner Physiotherapist to be the initial point of contact for patients with musculoskeletal conditions in primary care. You will assess, diagnose and manage patients, working autonomously alongside GPs and the practice team.

You will order imaging and tests, develop tailored management plans, provide exercise advice, and support self-management while liaising with MDT colleagues to ensure seamless patient care across services.

Qualifications

• HCPC registered physiotherapist with MSK experience.
• Experience diagnosing and managing varied MSK conditions.
• Proficient in GP IT systems SystmOne and EMIS.
• Proven ability to work independently in primary care.

Responsibilities

• First point of contact for MSK assessments in primary care.
• Develop integrated care plans with patient self-management.
• Order imaging, blood tests and referrals as appropriate.
• Work with GP team and MDT; maintain accurate records.
• Ensure safety and escalation when needed.
• Deliver education and training within the team.
