Voyage Care is seeking a dedicated Field Support Supervisor for a 12-month maternity cover at the Chapel Mews supported living service in New Whittington, near Chesterfield. The role supports six adults with learning disabilities, autism, and complex needs, delivering person-centred care and enabling meaningful community involvement.

You will work with the management team to maintain the highest standards of care, mentor peers, and ensure smooth service operation while empowering individuals to

Qualifications

• Previous experience in a supervisory or care management role.
• Level 3 Diploma in Health & Social Care (or equivalent).
• Experience supporting individuals with autism, acquired brain injuries and traumatic backgrounds.

Responsabilités

• Support and lead a team of care colleagues.
• Develop and implement person-centred support plans.
• Conduct care needs assessments.
• Induct, mentor and support new team members.
• Complete supervisions, appraisals and competency assessments.
• Undertake medication competencies and audits.
• Attend multi-disciplinary meetings and liaise with professionals.
• Complete reports and maintain records.
• Support day-to-day running of the service with the management team.
• Spend time with the people we support to help them achieve goals.
