Qualifications

• General education is required.
• Experience of working with the public is essential.

Responsibilities

• Visit households regularly, up to 120 in the area.
• Talk with people about health and wellbeing and advise on healthy choices.
• Identify problems and help find solutions; refer to health and social care services.
• Record visits on a tablet and share information with care teams.
