Hedena Health, a 25,000-patient GP practice, is recruiting a six-month community health and wellbeing worker to help people of all ages make healthier choices. The work is mainly field-based: regular home visits to up to 120 households, linking people with GPs, social workers and charities. Duties include health coaching and social prescribing, with visits recorded on a tablet. Fit rests on public-facing experience and local knowledge rather than a formal health qualification.

- Identify problems and needs and help find solutions
- Refer people to the right health, social care and voluntary services
- Support health coaching and social prescribing, helping people set and reach goals
- Work with GP surgeries, doctors, nurses, social workers and charities to support vulnerable people

- Build relationships with local community groups and volunteers, and support their work
- Help people understand and use health and social care services and manage their conditions
