Job summary

Caremark in the Winchester and Eastleigh area is seeking a Field Care Supervisor to implement high-quality, field-based care and support for customers and Care & Support Workers.

You will conduct needs and risk assessments, be on-call one weekend every month (paid extra), liaise with Care Coordinators, identify training needs, review client plans, and ensure timely reports and compliance while spending most time in the field.

Qualifications

1 year of Field Care Supervisor experience.
NVQ Level 3 is desirable.
Willingness to work in the Winchester and Eastleigh area.

Responsibilities

To carry out Individual Needs Assessments for new and potential clients.
To conduct risk assessments as part of the Individual Needs Assessment.
To be on-call (Paid Extra) 1 weekend out of 4 weekends every month.
To liaise closely with Care Coordinators regarding permanent allocation of Carer and Customer.
To identify any additional training needs of Care & Support Workers and report accordingly.
To conduct client reviews and respond to Individual Review Forms completed by Care & Support Workers.
To provide support to all Care & Support Workers in the field.
To monitor all clients for service and Care & Support Worker satisfaction.
To ensure that all written reports and Customer Compliance are up to date.
To maintain close liaison with both the Care Manager and Care Coordinator.

Responsible for implementing field-based quality care to the customers and providing support to all Care & Support Workers within Winchester and Eastleigh.

About the role

Principal responsibilities

To carry out Individual Needs Assessments for new and potential clients.
To conduct risk assessments as part of the Individual Needs Assessment and update these as required as part of the individual’s risk management.
To be on-call (Paid Extra) 1 weekend out of 4 weekends every month on a rolling calendar month.
To liaise closely with Care Coordinators regarding permanent allocation of Carer and Customer.
To identify any additional training needs of Care & Support Workers and report accordingly.
To conduct client reviews and respond to Individual Review Forms completed by Care & Support Workers.
To provide support to all Care & Support Workers in the field.
To monitor all clients for service and Care & Support Worker satisfaction.
To ensure that all written reports and Customer Compliance are up to date.
To maintain close liaison with both the Care Manager and Care Coordinator.

Field Care Supervisors are responsible for the efficient running of their own specific, defined areas and should vary their hours of work in order to effectively monitor and support all Care & Support Workers and clients.

Field Care Supervisors should spend approximately 80% of their working week in the field, with the remaining 20% spent in the office to complete necessary administrative duties.
